Permitting

  • The NCWOCICB has developed the following common form for use as a Notice of Intent to Construct a wastewater system. This form should be used for Authorized Onsite Wastewater Evaluators when providing a totally private permit

    Notice of Intent (NOI) Form

  • IP/CA Form for A2 Permits by AOWE, LSS or PE. This is commonly referred to as a Hybrid permit as the private licensed professional does the initial soil evaluation and design, and the local health department is the the final inspection.

  • This form is a common application for use with an application for IP or CA through a local health department using an A2 permit

  • The attached Notice of Intent to Construct is to be used for permits where a Soil Scientist and Engineer are privately permitting the wastewater system.

    EOP Common Form

  • A property owner may choose to bypass the local existing wastewater system approval process that is required in many counties when the owner wishes to add a “no flow” addition by submitting this signed affidavit to the LHD. It’s recommended that the owner submit this signed affidavit to building inspections, as well. Any damages incurred to the wastewater system as a result of the property addition are the responsibility of the homeowner.
